JUGGERNAUT FITNESS JUNGLE GROUP CLASS LIABILITY WAIVER

ACKNOWLEDGMENT OF RISK I understand that participation in Juggernaut Fitness Jungle Group Class involves physical activity including strength training, cardiovascular exercise, and conditioning drills. I acknowledge that these activities involve inherent risks of injury, including but not limited to muscle strains, ligament injuries, cardiovascular events, or permanent disability. I understand these risks may occur even when exercises are performed correctly.

VOLUNTARY PARTICIPATION & MEDICAL CLEARANCE I affirm that I am voluntarily participating and assume full responsibility for any risks or injuries. I certify that I am physically able to participate, have no medical conditions preventing safe exercise, and have consulted a physician if necessary. I agree to inform the trainer of any limitations before class.

RELEASE OF LIABILITY I hereby release, waive, and hold harmless Juggernaut Fitness Gym, its coaches, trainers, employees, and facility owners from all claims, demands, or injuries arising from my participation, whether caused by negligence or otherwise, to the fullest extent allowed by law.

EQUIPMENT & UNKNOWN RISKS I accept responsibility for using equipment properly and following all instructions. I acknowledge and assume all unanticipated risks that may arise during training.

PHOTO & VIDEO RELEASE I grant permission to Juggernaut Fitness Gym to use photographs or video recordings of me taken during classes for promotional purposes without compensation. (Note: If you do not wish to be filmed, please notify the trainer in person).

GOVERNING LAW & SEVERABILITY This agreement is governed by the laws of the state of operation. If any part of this agreement is found invalid, the remaining portions remain in full effect.

AGREEMENT I understand that I am giving up substantial legal rights, including the right to sue.

PRIVACY POLICY & DIGITAL TRACKING I acknowledge that Juggernaut Fitness collects digital data through its Webflow-hosted website. I consent to the use of tracking technologies, including Google Analytics 4 (GA4) and the Meta Pixel, which collect data regarding my website behavior and interactions. I understand this information is used for site analytics, customer tracking, and serving targeted marketing campaigns, including Meta (Facebook/Instagram) advertisements. By engaging with the website, booking services, and signing this waiver, I agree to this data collection and tracking.
